 These images were taken with a Lomography Diana F+ Instant Square camera with various lenses.

Ilford HP5+ Film, 12x20 Ultra Large Format

Here are some images made with 12x20 HP5+, developed in HC-110 dilution H (1:63), usually around 10 minutes at 70F.

Pentax 6x7 35mm f/4.5 Fisheye, adapted to 4x5 (Circular Fisheye)

The Pentax 6x7 35mm f/4.5 can be adapted to 4x5 to get a circular image - roughly 95mm wide. Some example images below:

This Schneider lens only covers 3 1/4" x 4 1/4" negatives, but can work on 4x5 if focused closely or if you don't mind vignetting. Here's some sample images:
